Net Control Stations should use this report form to submit their net reports to the net manager.
* NCS Call Sign
* Net Name  Worcester Emergency Net
 MEMA South Region 4 Net
 Other (Use comments to specify)
* Date of the net being reported mm/dd/yyyy
* Number announcements or traffic passed during the net
* List all net participant call signs
* Length of net in minutes
Comments for net manager (if any)
* denotes required field
